- Q: When do my benefits start over?
- A: Your Medical, Dental, and Vision benefits start over July 1 of every year.
- Q: When is Open Enrollment for Health benefits?
- A: You can make enrollment changes for Medical, Dental, Vision, and Flex any time between May 1 and May 31 of each year.
- Q: Do I need to enroll in Flex every year?
- A: Yes. In order to have a Flex account for the upcoming year your Flex enrollment form must be received in our office by May 31 of each year.
- Q: Do I need to enroll in Medical, Dental, and Vision every year?
- A: No. Your enrollment for Medical, Dental, and Vision will automatically be the same unless you decide to make changes during the Open Enrollment period for the upcoming benefit year.
- Q: What is an EOB?
- A: An EOB is an 'Explanation Of Benefits' that summarizes how your claim was processed.
- Q: Why do I need to keep my EOB?
- A: Your EOB should be kept in a safe place should you need to submit to your Flex account or for tax purposes. They will not be reproduced in our office because they are not generated by our system.
- Q: Why should I use the Mail Order Pharmacy benefit?
- A: Your prescriptions will be mailed directly to you. The deductible is waived on mail order only. Your prescription is a 90-day fill so you won't need to fill it as often.
- Q: Do I need a claim form with every claim I submit?
- A: No. The claim form has been replaced with an Annual Information Request form. Once requested it will need to be completed and returned once a year.
- Q: What is an Accident/Injury Information Request form?
- A: A request form used for each claim submitted with a diagnosis that indicates a possible injury or accident. Once requested it will need to be completed and returned as soon as possible in order to process your claim.
