Clerk of District Court - Forms

For making payments by credit card you can use the form in the following table entitled Credit Card Authorization.   By completing this form you are authorizing the Clerk of the Court's office to bill your credit card for charges incurred for child support, maintenance, judgments, searches, copies of documents, etc.  Once the credit card authorization form is completed, it may be faxed to 406-258-4899.
