Dissolution of Marriage (Divorce) & Family Law
The following forms have been made available online (by way of the internet through our website) for those who are seeking a Dissolution of Marriage ( also known as divorce) and/or Parenting Plan (also known as custody) and have decided against retaining an attorney for representation. If there are minor children involved in your case, you are highly encouraged to consider retaining the services of an attorney if you are able to afford one.
For those who decide to represent themselves we have provided procedural information for your convenience. This information contains detailed instructions on the procedures and steps involved. Please follow the steps carefully.

| D | Response to Dissolution Without Children | If you are responding to a divorce petition filed by your spouse, and do not have minor children |
| E | Dissolution with Children | If you are filing the divorce petition and you have minor children |
| F | Response to Dissolution with Children | If you are responding to a divorce petition filed by your spouse, and you have minor children. |
| G | Default Packet - Dissolution | If you have filed a divorce petition in Packet C or E and your spouse has not responded and you want his/her default entered |
| H | Petition for Permanent Parenting Plan | If you are not married to your child(ren)'s co-parent and you want to establish a parenting plan. |

| K | Transfer of Orders of Protection to District Court | If you have an Order of Protection from Justice or Municipal Court that needs to be transferred to District Court. |
| L | Filing Motions in Dissolution (Divorce) Cases | If you need to ask the Court to change something related to your divorce proceedings or final Decree. |
| M | Filing Motions in Parenting Custody Cases | If you need to ask the Court to change something related to your current custody proceedings or final Decree. |
