Program Description
The Be Your Best
Diabetes Prevention Program is a lifestyle balance program carefully
designed to help reduce the risk of developing chronic diseases such as
diabetes Type 2. You will learn how to lose weight and enjoy eating
healthy and being active. The program is based on research studies
regarding the best ways to help people make life-long changes that
support healthy eating, an active lifestyle, and weight loss and
management.
The classes are held weekly for the first 16 weeks, then monthly for 6
months. We cover nutrition and exercise in depth at the beginning, then
the remainder of the material is focused on making changes to your
lifestyle that support your weight and health goals. Weekly weights,
support meetings, plus food and exercise journals keep everyone on
track. We offer walks and exercise classes for the group as well as
passes to area gyms and studios to try new ways of exercise. We will
hold four sessions per year.
Are you interested? These are the steps you need to take in order to
enroll in this transforming program!
Step 1: Determine if you are eligible. You are eligible if you have
a BMI (body mass index) of 25 or higher plus any ONE of the following:

high blood pressure or taking medications for it
-
blood glucose between 100-125 (never been diagnosed with Diabetes) or taking Metformin
-
high cholesterol, triglycerides, LDL or low HDL or taking medication for it
-
having had a baby that weighed more than 9 pounds at birth
-
having had gestational diabetes when pregnant

Step 2: Get your FASTING blood work done - full cholesterol panel (LDL,
HDL, triglycerides) and blood sugar. If you have results within 6 months
of the first class, those are acceptable too.
Step 3: Your physician will then need to sign a
medical clearance
form. The lab results can either entered directly onto the
form or attached to it. This can be faxed to me or mailed - the contact
information is on the form.
Step 4: The last step is to meet with your life coach individually and
review your goals, plans, and challenges. I will be your life coach from
here on out, so it's important for us to get to know each other. You'll
have group support and individual support throughout the entire program.
